Welcome to VISTAS Geriatric Care Management Program.
Many people do not know where to turn when concerns arise with aging family members. Geriatric care managers, who are trained professionals that specialize in assisting older adults and their families, can be a valuable resource.
Our Program
VISTAS is a geriatric care management program of Family Service of Central Indiana, Inc. -- an organization that has been serving geriatric older adults for over 50 years.
Our experienced staff can help ensure that your loved one is:
- Living in a safe environment
- Receiving appropriate medical care
- Eating well-balanced meals
- Taking medications as prescribed
- Managing tasks such as dressing, toileting and bathing
- Engaging socially with family and friends
- Linked to services that meet his or her needs

Our Services
- Comprehensive in-home assessments
- Care planning to address older adults’ needs
- Coordination of appropriate, high quality services
- Assistance with admission and transition to a new environment
- Education, guidance and counsel for family members caring for older adults
- Communication with long-distance family members.
- Surrogate family visits and decision making for older adults without family support.
- Assistance with managing mail, paying bills and other financial matters
- Provision of guardianship and representative-payee services
